Veterans' Home Admissions Nurse
The Vermont Veterans' Home (VVH) is seeking an experienced and motivated Registered Nurse to work as our Admissions Nurse. The Admissions Nurse will work with a variety of referral sources (hospitals, home health care agencies, the VA, etc.) along with direct referrals from Veterans and their families. The incumbent will screen referrals for eligibility and gather all necessary information to make an admission decision.
This position requires responding promptly and professionally to all inquiries as well as educating community agencies, Veterans, and their families about the services provided at the home. They will follow the admission from referral to the physical admission and transition to the home.
The ideal candidate will have experience in admissions, preferably in long-term care), working knowledge of VA, Medicare, Medicaid and commercial insurances, VA, CMS, and State Nursing Home Regulations. Excellent customer service and organizational skills are required. Position requires participation in administrative weekend work rotation and general nursing tasks as needed. The Admissions Nurse reports to the Marketing and Admissions Coordinator.
Minimum Qualifications
Possession of (or eligible for) licensure as a Registered Nurse (RN) in Vermont OR eligible to practice in the state of Vermont via a multi-state license AND three (3) years or more of professional nursing experience in an acute hospital setting, long term care, health insurance carrier, or within a community health/public health setting.
NOTE: Must maintain Vermont or multi-state licensure as a Registered Nurse as a condition of employment.
