Behavioral Health Case Manager-Pediatric

PRIMARY FUNCTION

The Pediatric Behavioral Health Case Manager supports the care coordination needs of pediatric patients and families affected by mental health or substance use disorder. Works with patients and their families to develop and implement a person-centered care coordination plan, ensuring access to the supports and resources needed to improve mental health and health outcomes. Collaborates closely with other members of the WHC care team, as well as schools and other service organizations.

PRINCIPLE DUTIES

  1. Assess patient and family needs and develop patient centered care coordination plan that addresses patient/family’s social barriers to improving mental health and health outcomes. Provide close follow up and care coordination to ensure patient and family can access needed resources.
  2. Collaborate with other WHC care team members, school or daycare personnel, and other service providers involved with the patient.
  3. Provide health education to patients and families to reduce the impact of mental health condition on health outcomes.
  4. Serves as an internal reference for resources available to support pediatric mental wellness.

JOB REQUIREMENTS:

Minimum Education:  High School Diploma or GED. Bachelor Degree in Social Work preferred.

Minimum Experience: At least two years experience in a patient care setting.

TYPICAL WORKING CONDITIONS

Office environment with frequent contact with staff and the public. Work may include transporting patients/caregiver to appointments when needed in company vehicle.

ACCOUNTABILITY

Reports to Patient Support Services Manager.
